Childhood Asthma Control Test*

The Childhood Asthma Control Test is a way to help your healthcare provider determine if your child's asthma symptoms are well controlled. Share the results with your child's healthcare provider.

Answers to these questions are not saved on asthma.com. Only you can have access to your answers.

The Childhood Asthma Control Test is designed for children 4 to 11 years old. If your child with asthma is 12 years or older, or if you are an adult with asthma, take the Asthma Control Test™.

How to take the Childhood Asthma Control Test

  • Let your child respond to the first 4 questions (1-4). If your child needs help reading or understanding the question, you may help, but let your child select the response.
  • Complete the remaining 3 questions (5-7) on your own and without letting your child's response influence your answers. There are no right or wrong answers.
  • Complete the test by clicking 'SUBMIT'.
  • Take the COMPLETED test to the healthcare provider to talk about your child's score.
