Asthma Control Test™

The Asthma Control Test™ is a way to help you and your healthcare provider determine if your asthma symptoms are well controlled.

Take this test if you are 12 years or older. Share the score with your healthcare provider.

Step 1: Answer the 5 questions below. For each question, make sure you read all five possible answers before choosing the best one for you.
Step 2: Complete the test by clicking 'SUBMIT.'
Step 3: Take the COMPLETED test to your healthcare provider to talk about your score.

Answers to these questions are not saved on asthma.com. Only you will have access to your answers. If you wish, you can print the test and fill it out.

The Asthma Control Test is designed for adults and adolescents 12 years or older. If your child is 4 to 11 years old, take the Childhood Asthma Control Test* along with your child.

Now is a good time to make an appointment with your healthcare provider to discuss your score and ask about possible changes that may be needed in managing your asthma symptoms.

Remember, your asthma may change over time. Be sure to take this test periodically and work closely with your healthcare provider to monitor your level of asthma symptom control.
